Data Requests

A data request must be made in writing. There is no specific form required to request information about yourself or public data. However, two request forms are available for your convenience.

• Request for Public Data Form 
• Request for Data About Yourself Form  (to be used for requesting private data)

If you do not use the form, write a specific description of the information you seek. Written requests should include:

• A statement that you are making a request for information under the Minnesota Government Data Practices Act and,
• A clear description of the information you would like to access.

People can also obtain both public and private government data about themselves. The DCYF Guide for Requesting Data About You explains how to seek access to one's information. You may need to complete a Verification of Identify form to complete the request.

By law, DCYF Commissioner Tikki Brown is the designated authority responsible for the collection, use, and dissemination of the agency’s data. Data requests are handled by staff members assigned to respond to requests on behalf of the commissioner. Send data requests to:

Members of the media can send requests to the DCYF media relations team.

Data will be provided in a reasonable period of time. If you request data about yourself, you will receive a response within 10 business days of receiving the request, unless other arrangements are made for extra time.
