Minnesota Youth in Transition Database (MNYTD)
The federal John H. Chafee Foster Care Program for Successful Transition to Adulthood (the Chafee program) provides funding to and governs the independent living program known as STAY (Successful Transition to Adulthood for Youth) in Minnesota. The Chafee program requires a client-specific data collection system to track independent living services provided to youth, and defined outcomes that measure each state’s performance in preparing youth for successful transition to adulthood, from foster care to independent living.

STAY in the Community  
Community agencies provide individual independent living skills for former foster youth from age 18 up to 23 who are no longer working with a county or tribal worker. These services help youth prepare for a successful transition to adulthood. They also provide group independent living skills classes for current and former youth ages 14 up to 23
