  This application is used when a family first applies for a state Early Learning Scholarship at the Pathway II program serving the child.

• Early Learning Scholarship – Pathway II Renewal Form

  
  This renewal form is required to be completed for the annual renewal of an eligible child's Pathway II scholarship award.

• Early Learning Scholarship – Pathway II Supplemental Application

  
  Use this supplemental application to award scholarships when using a local enrollment form to replace the full scholarship application. This option is only available for school-based and Head Start/Early Head Start programs.
