U.S. Bank ReliaCard® stored value card account

• When a ReliaCard account is established to receive child support payments, the parent enters into a cardholder agreement with U.S. Bank. Parents who receive child support qualify automatically for this card. No credit check or bank account is needed.

  To sign up for direct deposit to a ReliaCard account, read the U.S. Bank ReliaCard® Disclosure and complete the U.S. Bank ReliaCard® enrollment form (PDF).

Although there is no fee for the card itself, U.S. Bank may charge fees on some account transactions, such as ATM withdrawals. U.S. Bank explains the fees in the packet they send with the card, and also on the cardholder website located at www.usbankreliacard.com.

Complete a Direct Deposit Authorization Form (PDF) to change your direct deposit from a ReliaCard account to your checking or savings account. When you are set up for direct deposit to checking or savings, your ReliaCard account will automatically be closed.

If you need to close a ReliaCard account because your child support case is closed, the balance on your card must be $0.00. When your balance is $0.00, call the state child support help desk to close your ReliaCard account. Closing your child support case does not automatically close your ReliaCard account.

Completing a new direct deposit application will also close the ReliaCard account.
