Do you have questions about the new Family Outcomes Survey form or process? Please join us for open office hours hosted by the Minnesota Department of Children, Youth, and Families with the Minnesota Department of Education.

These informal open office hour sessions are designed to support districts and cooperatives as they navigate the updated FOS collection process.

Whether you have questions about:

• Distributing electronic or paper surveys to families
• Reading and using your monthly FOS spreadsheet
• Understanding Part C exit codes and MARSS reporting
• Supporting families who need translated surveys or cultural liaison support
• Or anything else related to FOS implementation
